Community Choices

Welcome to Community Choices -- your aging and disability information source.

Community Choices is a partnership between the Center for Disability Rights and the Agency on Aging of South Central Connecticut. It is a single point of entry for both folks with disabilities of all ages and older adults. It provides information to empower older adults and people with disabilities to advocate for themselves.

In Connecticut Call (800) 994-9422. When you call, please state that you are calling for the Aging & Disability Resource Center or ADRC. Our specially trained staff will be happy to assist you Monday through Friday 8:30 a.m. to 5 p.m.

Community Choices is made possible by an Administration on Aging grant through the State of Connecticut and is staffed by the Agency on Aging of South Central Connecticut and the Center for Disability Rights. Both organizations are at the forefront of promoting empowerment and self-reliance for older adults and persons with disabilities.Community Choices logo
